Before You Register Online:

Before you proceed to the online registration form, please take a moment to collect the following information:

1. •Home, mobile, and work phone numbers for the student’s parents and stepparent (if applicable).
   

2. •Email addresses and mailing addresses for the student’s parents and stepparent (if applicable).
   

3. •Your child’s class choices. Please be sure that their grade as of Fall 2010 corresponds to the eligible grade range.
   

4. •Your child’s physician’s name and phone number.
   

5. •A list of your child’s food, medicine, and substance allergies.
   

6. •A list of your child’s regular medicines and dosages.
   

7. •Your child’s health insurance information.
   

8. •Names and phone numbers of two emergency contacts.
   

9. •Names, driver’s license numbers, and phone numbers for up to 4 people who are authorized to pick up your child from Summer Wonders.
   

After you have submitted your online registration form, you will see a message that looks like this:

If you do not see this confirmation screen, the registration was not successfully submitted.

After you submit your registration form, we will contact you by email to confirm your registration and send a billing statement.
