Required Application Materials

In addition to completing the online REGISTRATION FORM and submitting the online TEACHER RECOMMENDATION, all applicants must submit the following materials.

Note: Acrobat Reader (free from Adobe) is required to view the Behavioral Contract form below.

1. •Behavioral Contract (must be signed by parent and child)
   

2. •Current photo
   

3. •Copy of your child’s current immunization form (please make sure your child’s name is visible at the top of the form)
   

4. •$100 Deposit per session (if submitted before May 1st), or Payment in full.
   

Please download and sign the Behavior Contract and submit all of the materials above together in one envelope, mailed to:

Application Reminders

1. •The Registration Form and Teacher Recommendation must be submitted online.
   

2. •We do not accept applications or payment by phone.
   

3. •Submit payment by check made out to Summer Wonders.  We do not accept credit cards at this time.
   

4. •Please make sure the copy of your child's immunization history is clearly labeled with your child's name at the top.
   

5. •Please follow up with your child’s teacher to make sure they’ve submitted their online recommendation.
   

6. •Remember to review the Behavioral Contract with your child!  Your child's signature must be on the contract in order for your application to be considered complete.
   

7. •Incomplete applications will not be processed.
   

8. •Please submit complete applications by the appropriate deadline.  If openings are available, late applications will be accepted with a late registration fee for each session until the late application deadline.
